Act of Donation: Close Family Relationship

Used to donate a vehicle from one closely related individual to another

Designated Handicap Form

Used when a handicap individual allows another individual to put his/her handicap plate on a vehicle being purchased.

Federal Odometer Statement

Used to disclose mileage on a vehicle being purchased. Must be signed by buyer and seller.

Lease Exemption Form

Used to allow exemption of sales tax at time of registration with current dealer permit numbers and lease exemption numbers for lease and/or rental vehicles.

Limited Power of Attorney

Used to allow a third party to sign documents for the owner/buyer of a specific vehicle.

Physician's Certification of Mobility Impairment

Needed to purchase a handicap plate. Form must be completed by a physician stating the handicap status of Permanent in order for a handicap plate to be issued.

Request for Commercial Plate

This form must be completed in order to obtain a commercial plate for a private auto registered in the name of an individual

Title Authorization

Used to allow title to be mailed to a third party.

UCC1

Used as a lien recording document.

VEH (1799) Vehicle Application Form

Must be completed for all Louisiana vehicle transactions.
