  1. Louisiana Application (VEH, 1799)
  2. Certificate of Origin
  3. Bill of Sale/Invoice
  4. Odometer Statement
  5. Any Affidavits
  6. Proof of Trade (copy of title or registration)
  7. Copy of Security Agreement (if applicable)

Miscellaneous Affidavits based on ownership of the trade:

Affidavit of One in the same: Their name may be somewhat different on the trade such as a maiden name or sometimes goes by middle name but they are the same person. Must state reason why they are one in the same.

Act of Donation: This is a separate transaction and must be completed in order to register the trade vehicle in the new owners name in order for that person to receive trade (tax) credit. Donation form must be notarized.

Affidavit of Permission To Trade: This is used for someone to give permission to his or her spouse to trade a vehicle that is in one spouse's name without registering that vehicle in the other spouse's name to receive trade (tax) credit.

Affidavit of Sole Owner: This is used when an individual is doing business as (D.B.A.) his or her Co. and wants to receive trade (tax) credit for a vehicle being titled in their personal name or visa versa.

Affidavit of Error: Used whenever a clerical error is corrected on a crucial document in the deal. (do not use to correct the complete VIN, Name, or Money errors)

NOTE:
ALL AFFIDAVITS MUST BE PROPERLY WITNESSED PRIOR TO BEING NOTARIZED.
